In return for a service commitment, the program pays a substantial sum toward your loans, with the option to extend service for additional repayment. It is one of the largest direct-repayment programs in healthcare.
NURSE Corps and other federal programs
- The NURSE Corps Loan Repayment Program pays a large share of nursing debt for registered nurses, nurse practitioners, and nurse faculty who serve in critical-shortage facilities or eligible schools of nursing.
- The Indian Health Service offers loan repayment for clinicians serving American Indian and Alaska Native communities.

Sequencing with PSLF
Many of these clinicians also work for nonprofit or government employers, which means PSLF is on the table too. The key is sequencing. Program payments that pay down principal directly are best used early, while PSLF rewards keeping a balance large through minimal IDR payments so a bigger balance is forgiven tax-free at year ten. Mixing them carelessly — making large payments during PSLF years, or burning PSLF-eligible time — can quietly waste five figures. Map the ten-year sequence before you commit.
